Role Description

This is a full-time, on-site role for a Nurse Case Manager located in Las Vegas, NV. The Nurse Case Manager will be responsible for discharge planning, utilization management, nursing care, and overall case management. Daily tasks include assessing patient needs, coordinating care plans, collaborating with medical staff, and ensuring seamless transition of care for patients. The Nurse Case Manager will also play a key role in improving patient outcomes and maintaining regulatory compliance.

• Excellent communication and interpersonal skills
• Ability to work independently and as part of a healthcare team
• 3 years of experience in a hospital setting
• Bachelor's degree in Nursing (BSN) or related field; RN license required
